How can I set my desktop context menu (the menu you get when you right click on the desktop or and Explorer window) to have the option to create a new txt document? Mine seems to have dissappeared.

You can do this with Microsoft's Powertoys (TweakUI).... what OS are you running?
If you have (or know how to get) TweakUI installed on your system, create an empty text (.txt) file on your desktop (just create a new, blank doc in Textpad then save it as blank.txt or something). Launch TweakUI, and then go to the New tab. Drag that text document into the list, and bada bing.
Actually, before yo go to the trouble, you should just go into the New tab of TweakUI and see if the item is already there, and simply unchecked. Then all you've gotta do is check the box, Apply, and you're there.
