checkboxes & radio buttons on one program

i am trying to combine checkboxes and radio buttons on 1 java program using text pad, html applet etc...
the applet quikly comes and disappears, and the browser shows a horz line and a vertical line only.

Help how do i combine 2 objects on one progrm and they show up on the browser
