Wow, this was posted over a year ago, but there doesn't seem to be any resolution. I am using 4.7.3, and I have the same problem (at least 1 and 2) which is driving me nuts!

I do not have to do step ii to recreate the problem. I can just start a new file with an extension of a type that saves hard ...
