Search/Replace problem: only ONE occurrence for "Replace All"

When I click "Replace All", only ONE occurrence is replaced.

The only way ALL occurrences are replaced is when I do Edit/Select All first then click "Replace all"

What am I doing wrong?

Make sure either nothing is selected or your current selection includes a newline.
