Why does Find All and Replace All NOT find/replace ALL occurrences of the what is in the "Find What" box?

I have to first do Edit/Select All then click Find All or Replace All

What do you have selected in the scope section of the replace dialog?
Is it "selected text"?

If so, switch to "active document".

active document is selected

all documents is not seleccted
