Actually, the operation on my Windows 7 system seems to be a little different than you describe. However, setting BOTH "Replace Notepad" AND "Allow multiple instances to run" gets me where I want to be: new window every time, no unexpected results, no guessing.
Thanks to all who have helped ferret out how these two settings interact and how they are both required to be set in certain ways.

I have no issues running multiple volumes, I have basically only configured it to use the (replace notepad.txt & line numbers).
Me thinks you might try a complete uninstall reinstall? And wipe out the upgrade/migrated options/preferences between those versions.
Been working fine for years here...
