File Explorer sends DDE messages with the file name to TextPad when you double click a file, whereas is starts a new instance with the file name in its command line when you right click and and choose Open with TextPad. Maybe another application is delaying the DDE messages?

This is a bug which will be fixed in the next release. Thanks for reporting it.
In the meantime, you can work around it by setting the default scope of the macro to "Document", rather than "Selection".

First off I'd like to reassure you that the only time TextPad accesses our server is when you run "Check for Updates" from its Help menu. Even that can be disabled using policies as described in the help file. It's not clear to me whether the slow behavior you're seeing is during the load ...

As Amigo Jack points out, if you are using the themed file open dialog box, this could be the issue, as it can be slow to open because it doesn't share the cache used by the system file open dialog. If TextPad cannot save to a file, perhaps because it's read-only, or its timestamp has changed since it was opened, or the original no longer exists, the Save As dialog box is displayed to alert you.
